Welcome to IPHCA Monthly for August 2025!

This month, we are pleased to announce a new partnership with the Indiana Department of Health (IDOH) for their Learning Collaborative, with Chronic Disease Management serving as the central theme for this year’s work. This collaboration will bring valuable resources, shared expertise, and innovative strategies to support health centers in improving outcomes for patients managing chronic conditions. See page 10 – 11

We are also looking ahead to our Annual Conference presented by AndHealth, taking place this October 7 & 8 in Terre Haute. Check out the full speaker lineup, and download our conference app, designed to help attendees plan their schedules, connect with colleagues, and maximize their experience! This year’s program promises engaging sessions, thought-provoking keynotes, and practical tools to support whole-person care in our communities. See pages 4 – 9

Finally, we are ready to celebrate National Health Center Week, August 3–9. This week is not only a chance to showcase the vital role of community health centers but also to pay tribute to the dedicated health providers across Indiana who work tirelessly to keep our communities healthy.
