DESCRIPTION:A special session at IPHCA 2024 \nWednesday\, October 9 /  8:30 am – 11:30 am \nThis interactive session focuses on Value-Based Care and the Clinically Integrated Network.  You will hear from the Texas and Ohio PCAs about their real-time experience in establishing and beginning operations of their CIN\, learn from experts about data use\, and have the chance to discuss it directly with the MCEs. \nLearning Objectives \n\nRealistic understanding of operations in the initial years of the organization\nThe importance of the use of data in Value-Based Care and understanding how we analyze and synthesize different types of information\, including some tools\, tricks\, and pitfalls learned from hundreds of integrations across the country\nInteractive discussion with the MCE\n\nThis event is only available to attendees at our Annual Conference – click here to register \n\n  \nThis special session is in three parts: \n\n• Health Plans as Partners: Indiana’s Medicaid Managed Care Entities \nSpeakers: Mackenzie Blevens and Amy Murray (Anthem)\, Dr. Cameual Wright\, CMO (CareSource)\, Jessica Cromer\, CEO (MDwise)\, Jill Claypool\, VP Network Development and Contracting (Managed Health Services) \nDescription: In this session\, you will hear from Indiana’s Medicaid Managed Care Entities (MCEs). The MCEs see health centers as partners in our shared goals for health equity. They will share their vision for and their commitment to working with the Clinically Integrated Network of Indiana. You will hear their quality and access priorities and how value-based payment arrangements with health centers can enable innovative responses to those shared priorities. Learn how Indiana’s MCEs are coming together with each other and health centers to find solutions to our common challenges. \n• Stories From the Field: The Early Journeys of Clinically Integrated Networks in Ohio and Texas \nSpeakers: Heather Simpson\, Director of Value-Based Programs (Ohio Association of Community Health Centers\,\nFranchella Jennett\, Executive Director\, Texas Association of Community Health Centers Clinically Integrated Network \nDescription: In this session you will hear from two states who have recently formed Clinically Integrated Networks. Each state has unique journeys to share. While we have heard many times from some of the pioneering states\, now is the time to hear from others who are just a year ahead of us on their journeys. They will share their experiences launching their CINs and how they are building the infrastructure to support health centers to perform in new payment arrangements and they are in the first leg of the value-based payment marathon. \n• Understanding and Using Data in Value-Based Care \nSpeaker: LuAnn Kimker\, VP\, Clinical Innovation (Azara Healthcare) \nThe current healthcare landscape generates more data than ever before from a variety of sources: EHR clinical information\, HIE transitions of care events\, payer claims\, rosters\, care gap files\, wearables\, etc. Combining this information into one platform gives users a richer picture of their population’s cost and care metrics; the US healthcare system\, however\, allows for such a diversity of information formats and sharing norms that even when it is all in one place it can be challenging to interpret and use that data. IPHCA has been setting the foundation for a population health strategy that supports the transition to value-based care that considers all available data\, the strengths of each set\, and how to bring them together (including deciding not to). In this session we will explore what the leadership team needs to know about how Azara Healthcare analyzes different types of information and some of the tools\, tricks\, and pitfalls we have learned from hundreds of integrations across the country.
