Implementing SDOH screening is only half the battle when it comes to responding to patients’ social determinants of health. Health centers who collect SDOH data from patients need a reliable platform to connect patients to services and community-based organizations. Participants will hear from a panel representing various referral service platforms who have worked closely with CHCs to respond to social risks and close the referral loop.
