This year’s theme, “The Heart of Health Care,” celebrates the central role that community health centers and frontline providers play in delivering compassionate, comprehensive care to Hoosiers across the state.
The conference brings together professionals from across Indiana passionate about improving access to and quality of primary care. This year’s event will focus on the people and practices that keep communities healthy, emphasizing collaboration, excellence, and sustainable care models.
We invite you to share your knowledge, highlight innovative practices, evidence-based models, and impactful stories from the field. We welcome your expertise in clinical integration, behavioral health, value-based drivers of health, clinical operations, workforce development, policy, or other areas that advance primary care.

Our theme, The Heart of Health Care, honors the people, purpose, and passion that drive community health forward.
Community health centers are more than providers. They are trusted anchors within the communities they serve. This theme reflects these centers and their teams’ essential role in delivering compassionate, patient-centered care that meets people where they are. It also emphasizes the human connection at the core of primary care. Whether through a provider’s clinical expertise, a navigator’s outreach, or a leader’s strategic vision, in the current dynamic environment, it is vital to return to what matters most: relationships, trust, and the health of the whole person.

We provide free registration to the conference and, if necessary, can provide one-night lodging and in-state mileage reimbursement for presenters from CHCs and other PCAs.
