SDOH Champions Workgroup
The SDOH Champions workgroup is for key staff and personnel dedicated to advancing their organization’s Social Drivers of Health (SDOH) initiatives. This is an interdisciplinary group that includes; Clinical team, Quality team, Outreach and Enrollment team, and Leadership team members.
SDOH Champions is designed to assist health centers as they collect and respond to social risk data and to promote leveraging technology to reduce health disparities. The workgroup functions as a resource-sharing platform for participants to learn about and discuss interventions that connect patients to SDOH services.
The group meets once every six weeks in a peer call format to hear from content experts, share best practices, and discuss challenges and solutions.
