Director of Value-Based Programs

Hannah Putnam-Johnson

Hannah Putnam-Johnson supports health centers statewide in advancing population health management, practice transformation, and value-based care in her role as Director of Value-Based Programs. Hannah directly supports the Clinically Integrated Network of Indiana (CINI), a wholly owned subsidiary of IPHCA. Her work includes implementing performance improvement strategies focused on total cost of care, contract management, and educating health centers on the intersection of Patient-Centered Medical Home (PCMH) and value-based models. Hannah also oversees data analysis projects, supports governance through facilitation of board and committee meetings, and contributes to grant development and strategic planning. She brings over five years of experience in healthcare transformation and data management.

Hannah holds both a Master of Health Administration (MHA) and a Master of Public Health (MPH) from the Indiana University Fairbanks School of Public Health, as well as a Bachelor of Science in Public Health from Purdue University. Before joining IPHCA, Hannah completed fellowships with Eskenazi Health, where she supported quality improvement initiatives for perinatal depression screening, and with the Indiana Rural Health Association, where she developed a policy proposal promoting food justice for rural youth. Her healthcare career began in 2015 as an EMT-B in rural Indiana, which inspired her long-standing commitment to health equity.
