Quality Improvement Coordinator
Funke Oladele obtained a Bachelor of Science Degree in Psychology-Clinical Rehabilitation from IUPUI and a Masters of Business Administration with Health Management focus from Indiana Wesleyan University. Funke has been in the healthcare field for about nine years focusing on Care Coordination, Case management, Practice management, Quality Improvement, and Wellness Coaching. She spent more than five years working at Eskenazi Health as a Care Coordinator in one of the Community Mental Health Clinics, coordinating care and resources for the underserved population with mental illness; she has also worked as a Doctor’s Practice Manager, managing business operations and leading quality improvement initiatives, and as a Health Coach with a health management company providing lifestyle Coaching to Corporate Health Plan Recipients
Role at IPHCA
Coaches and conducts performance assessments with PHCs related to HCCN QI projects and initiatives
Facilitates and Support HCCN’s participating health centers in workflow redesign using lean methodologies.
Supports participating health centers to continue to focus on clinical quality measures.
Assists IPHCA/FQHCs and other health center members with Patient-Centered Medical Home Recognition
Provides support to participating health centers on Social determinants of health and promote PRAPARE.
