Indiana PRAPARE Peer Learning Collaborative

IPHCA's Goals for the Indiana PRAPARE Peer Learning Collaborative include:

- Coaching and engaging health centers to better understand patient populations and how to respond to needs.

- Scaling PRAPARE tool implementation statewide

- Leveraging aggregate data to inform policymakers and influence payment reform efforts

- Being an ongoing resource to health centers looking to address social determinants of health through PRAPARE

The collaborative includes webinars, office hours, in-person training events, and personalized technical assistance.
