Applying for Medicaid


Ready to Apply for Medicaid?

Depending on your household income, age and medical needs, you may be eligible for Medicaid, or an Indiana Health Coverage Program (IHCP) like Hoosier Healthwise or the Healthy Indiana Plan (HIP). To find out if this may be an option for you, use this eligibility guide or this screening tool. You can apply by one of the options below any time of the year, and help is just a click or call away
Preferred method: Apply online. Visit this page and select “Apply for Health Coverage, SNAP, and/or Cash Assistance online." This application will determine eligibility for all Indiana Health Coverage Programs such as Hoosier Healthwise, the Children's Health Insurance Plan (CHIP), and the Healthy Indiana Plan (HIP). If you're unsure if you qualify for Medicaid, you can also apply and shop on the Marketplace

Apply by phone. Call 1-800-403-0864 between 8:00am-4:30pm (local time), and a staff member of Indiana's Department of Family Resources (DFR) will assist you with your application. If you are interested in the Healthy Indiana Plan (HIP), please call 1-877-438-4479. 

  Apply in Person. Visit your local Department of Family Resources (DFR) office. Find your local DFR office on this page by selecting your county. You can also locate an Indiana Navigator in your county by using this page 

Apply by mail. Print an application here, and mail it to the address below. Make sure you select the box “Health Coverage” and complete all required information. Answering each question may expedite your application process.
