Please join Health Outreach Partners as they present on the Social Determinants of Health.  Structural conditions in which people are born, grow, live, and age can influence their health and well-being often  more significantly than genetics and individual behaviors. Communities served by health centers are particularly vulnerable to poor health outcomes due to these structural conditions, or Social Determinants of Health (SDOH). SDOH  include lack of or inadequate transportation, poverty, and difficulty navigating the complicated health system. Needs assessments are one way to help health centers better understand SDOH in their own communities and create responsive programs and/or services. During this webinar, HOP will provide the definition of SDOH and the impact on healthcare services. HOP will also link SDOH with top barriers to health insurance enrollment, particularly in rural areas.  Finally, participants will learn the community health needs assessment process, resources to support health centers in providing needs assessments, and examples of innovative practices.
