Risk Stratification and Hypertension
Risk Stratification is a technique for categorizing patients based on their health status which allows CHCs to manage patients based on assigned risk level. In this FoQ session, we will discuss how to develop a risk stratification model while reviewing an existing risk stratification algorithm. This enables CHCs to determine how effective their current risk model is and ways to better utilize it. We will also have one of the Million Hearts Champion health centers sharing best practices on Hypertension as it relates to Medication reconciliation and management.
