Focus on Quality: Transformation Series
The Indiana Primary Health Care Association (IPHCA) and the Indiana Quality Improvement Network (IQIN), a health center controlled network (HCCN), have partnered to streamline quality improvement training efforts for community health centers through a learning collaborative. IPHCA and IQIN share a goal to improve the performance of clinical and operational quality measures using data, health IT, and implementation of promising practices around Indiana.
Focus on Quality: Transformation Series is a monthly learning collaborative organized by IPHCA and IQIN to support Indiana’s community health centers (CHCs) quality improvement efforts in moving along the value-based Continuum. The collaborative covers trainings and resources from content experts, sharing of promising practices from CHC peers, and peer interactions.
