Care Coordination and Referral Management

This webinar will focus on the fundamentals of Care Coordination and how to set up an effective care team structure. Effective care coordination involves organizing patient care activities and information among all participants involved in inpatient care. This requires timely availability of patient information to the right people to guide the delivery of high-quality care.

Candice Chitty is the CEO of Quality First Healthcare Consulting, Inc. (QFHC) Chief Operating Officer of Quality First Management Solutions, Inc. (QFMS). Candi brings over 35 years of experience having held staff, management, and executive-level positions in hospitals, physician groups, home health agencies, and health plans. Ms. Chitty has worked with HRSA’s grantee programs since 2003. The assistance she provides includes clinical services technical assistance (TA) expert, FTCA application reviewer, special projects consultant, and operational site visit, consultant.

At the end of the session participants will be able to learn:

  • Steps to consider in assessing your care coordination activities and making improvements.
  • Key obstacles and critical success factors to consider when implementing care coordination and care-management model.
