Age-Friendly Care Mini-Fellowship – Virtual Series

IPHCA is delighted to announce the launch of our newest educational Lunch and Learn, the “Age-Friendly Care” Mini Fellowship presented by Drs Patrick and Diane Healey, taking place over 6 sessions between April 29 and July 15.

The Healeys have a combined 70years of clinical experience as fellowship trained geriatricians in the central Indiana region with teaching and program development interests. While now retired from clinical work they are still enthusiastic about the newly developing field of age friendly care for seniors.

In today’s healthcare landscape, understanding and catering to the unique needs of older adults in primary care settings are more crucial than ever. This mini fellowship aims to equip our healthcare professionals with the knowledge and skills necessary to provide comprehensive and effective care to our aging population.

Why is understanding the needs of older adults in primary healthcare so important? As our population ages, we are presented with both opportunities and challenges. While advancements in healthcare have led to longer life expectancy, they have also brought about an increased prevalence of chronic conditions and complex care needs among older adults. Therefore, it is imperative that we adapt our approach to care to meet the evolving needs of this demographic.

The “Age-Friendly Care” Mini Fellowship will provide participants with a deep dive into the concept of age-friendly care and the 4Ms framework, which emphasizes the importance of addressing What Matters Most to older adults.

Here’s a glimpse of the proposed agenda:

Week 1: April 29th 
Overview of changing older adult care with the concept of age-friendly care and the 4Ms.
Begin introducing the discussion of determining clients’ values and goals.

Week 2: May 13th
What Matters: Discussion of advance care and current care goal setting.

Week 3: June 3rd 
Medications: Discussion of prescribing and deprescribing of medications for older adults.
Register for Week 3

Week 4: June 17th 
Mentation: Awareness of dementia, depression, and delirium.
Register for Week 4

Week 5: July 8th 
Mobility: Understanding the importance of mobility and function in older adults.

Week 6: July 15th
Tips on billing/Resources/QI project presentations.
Register for Week 6

Each week will feature interactive sessions, case studies, and discussions led by experts in the field. Participants will not only gain a deeper understanding of age-friendly care but also practical skills that they can immediately apply in their clinical practice.
