Membership Info
There are 7 classes of membership in the Indiana Primary Health Care
Association (IPHCA), as described below. In order to qualify as
a Primary Care Provider member (categories A – C, below), the organization
must:
- Systematically require meaningful input from the community of patients
it serves on all significant medical services delivery issues;
- Charge for services according to a sliding fee schedule based on Federal
Poverty Guidelines;
- Provide primary care health services for all patient life cycles;
and
- Demonstrate a commitment to serving as a "safety net" medical provider
for its community.
Specific membership categories are as follows:
- CONSUMER GOVERNED PRIMARY CARE PROVIDER: Any
not-for-profit or tax exempt organization committed to the mission of
IPHCA (as stated in the Articles of Incorporation) and:
- Actively engaged in providing primary health care;
- Governed by a Board of Directors comprising more than 50% consumer
/ community membership; and
- Providing services to medically under-served persons on a sliding
or reduced fee scale.
- STATE-FUNDED COMMUNITY HEALTH CENTER PRIMARY CARE PROVIDER: Community
Health Centers governed by a Board of Directors comprising less than
50% consumers and receiving operational funding through monies designated
for Community Health Centers by the State of Indiana.
- ASSOCIATE PRIMARY CARE PROVIDER: Any not-for-profit or tax exempt
organization committed to the purposes of IPHCA and offering primary
health care services to medically under-served persons on a sliding
or reduced fee schedule.
- AFFILIATE: Any not-for-profit or tax exempt organization conducting
business on behalf of improved access to primary health care services
for Indiana residents, but that does not itself provide such services.
- STUDENT / RESIDENT: Anyone in a health
care trainee status who supports IPHCA's mission.
- INDIVIDUAL MEMBER: Any other person supporting the
mission of IPHCA, except:
- An employee / Board member / contractor of an organization eligible
for any Primary Care Provider membership category (as noted above)
that is not an IPHCA organizational member; or
- An employee of an IPHCA funding / granting agent or contractor.
Individual
members may serve on any committee or be elected to the Board.
- CORPORATE
PARTNER:
- Any for-profit vendor or potential vendor of
a Community Health Center or other "safety net" primary care provider;
and
- Any other company, organization, or independent business person
supporting IPHCA's mission, whose business or organizational interests
do not conflict (either in practice or in appearance) with IPHCA's
philosophy, culture, image, funding agency requirements, or purpose
of improving health and fostering the provision of high quality
primary health care services in Indiana.
