Jasmine Page, IPHCAs Healthcare Data Specialist will be discussing PRAPARE, and share why it is important and how you can be involved. Caresource Medical Director, Cameual Wright, MD, MBA will be talking about working with the Dept of Corrections to create a strategy to meet the needs of the population re-entering society from incarceration. Caresource is piloting pre-release engagement with members to identify their needs and begin to formulate a plan of care.

"CareSource and the Re-Entry Population in Indiana”

CareSource is working closely with the Department of Corrections to create a strategy to meet the immense physical, behavioral and social needs of the population re-entering society from incarceration.  Amongst many planned interventions, we are piloting pre-release engagement with members to identify their needs and begin to formulate a plan of care.  We hope to partner with the FQHCs, where many of these members will seek care, to streamline processes and foster care collaboration.

The presentation will provide an overview of the PRAPARE project, the types of data collected, and how the data impact patient health. IPHCA has made a commitment to assisting health centers in collecting and responding to patient identified social determinant of health data. Learn about how your health center can begin collecting this data as well.

Acts as clinical liaison to the state, providers and local trade associations.  I am accountable for the clinical initiatives that impact our quality metrics and goals.

Prior to joining CareSource, I practiced Obstetrics and Gynecology in Indiana for 15 years.  I completed my undergraduate, medical school and residency training at the University of Michigan in Ann Arbor.  I also obtained a Master in Business Administration from Indiana University. When I’m not working, I love to travel the world and relax with my 4 dogs.
