"The Medical Home: Disruptive Innovation for a New Primary Care Model"

A new report from the Deloitte Center for Health Studies addresses the growing concept of the "medical home."  This is a model in which primary care clinicians and allied health professionals provide not only continuing hands-on health services, but also coordination of the full spectrum of care needed by their patients.  In many respects, this is the model employed by Community Health Centers.

The medical home is increasingly being seen as a way to both improve the nation's management of chronic conditions and decrease health care costs.  Nearly half of the US population needs chronic care, and an estimated 25% of Medicare patients have at least five co-morbidities.

This new report addresses medical home models, their potential for generating significant savings to the US health system, and long-range policy implications.  In addition, this balanced review assesses business risks and up-front expenses for medical practices considering moving toward this model.
