Below is a listing of Community Health Centers in Indiana, keyed to the accompanying site map. Included are all Federally Qualified Health Centers (FQHCs) and Indiana's State-Funded Health Centers.

In addition, in mid-2007 it was announced that Indiana had been chosen to receive Federal funding for three one-year planning grants to develop new FQHCs in Newton, Carroll, and Posey Counties.    

Also in 2007, NorthShore Health Centers received an Expanded Medical Capacity grant for Portage, as well as a New Access Point grant for Lake Station.
